Check-ups, disease management, illness or injuries

Your Primary Care Provider is the best place to receive care for preventative care, disease management, and routine illnesses or injuries.   You can phone your preferred office to schedule an appointment. For those who are signed up for the patient portal, we do offer the ability to request an appointment online. To use this option now, click here – Patient Portal Login

Some same day and next day appointments are available for established patients at each of our locations. We want to be sure that we are available when you need us!

Same Day appointment times are available for non-emergency care for things like:

  • strains and sprains
  • scrapes and cuts
  • minor burns
  • fevers
  • coughs, cold/flu
  • rashes

Established patients can request a same day or next day appointment by phoning the location where they normally receive care.  You may access the Patient Portal to request an appointment, but we recommend calling us if a same day appointment is needed.
