Our Mission

To provide patient-centered, quality healthcare services regardless of the ability to pay.

Our Vision

To be the lead provider of innovative, quality healthcare services.

Our History

Originally established in 1978 in Lincoln, MI, Alcona Citizens for Health Inc, also known as Alcona Health Center, is a non-profit, Federally Qualified Community Health Center with locations throughout northern lower Michigan and the eastern upper peninsula. We provide comprehensive medical, behavioral health, dental and pharmacy services with locations in the counties of Alcona, Alpena, Cheboygan, Chippewa, Iosco, and Emmet.

All Patients Welcome

Alcona Health Center welcomes all patients. We do not discriminate, exclude people, or treat them differently based on race, color, national origin, age, disability, religion, sex, or source of payment.

Patient-Centered Medical Home

Alcona Health Center adheres to evidenced-based guidelines to provide the highest quality of care to our patients. We are an accredited Patient-Centered Medical Home.

A Patient-Centered Medical Home uses a team approach to providing healthcare. Your healthcare team includes your primary care provider, other medical staff, those that support you and most importantly – YOU!

5 Joint Principles for a Successful PCMH Model

Comprehensive Care

This is provided by looking at you as a whole person, not just focusing on one problem. It also means having a team to help with your mental and physical total health.

Patient-Centered

This centers on building strong relationships with patients and their families; understanding their backgrounds, beliefs, and what they need from healthcare is essential.

Coordinated Care

Coordinated care is delivered by ensuring everyone involved in a patient’s care knows what is happening. This might involve sharing health information between different places where the patient gets care including hospitals and clinics.

Accessible Services

These are distributed by having flexible hours, offering different ways to get care in emergencies and using technology like video calls to connect with patients.

Quality & Safety

Created using the best methods and tools to ensure patients receive safe and effective care that meets their needs.

Board of Directors

Alcona Health Center’s volunteer Board of Directors is composed of our patients and other representatives from the geographic areas that we serve. Their role is to provide oversight and strategic direction for our organization. They ensure that we are holding true to our mission and that our services are meeting the ever-changing healthcare needs of our Northern Michigan communities.
