At Alcona Health Center, we are committed to providing complete, personalized care that meets the diverse needs of our community. We work hard to fill gaps in care through the dedicated support of care managers, community health workers, transition of care coordinators, and care connectors. Our team ensures that every aspect of your health is addressed—not just your immediate medical concerns, but your overall well-being. In addition to one-on-one care, we offer nutrition classes led by a registered dietitian, providing expert guidance to help you make healthy choices and manage conditions like diabetes, heart disease, and obesity. At Alcona Health Center, your health journey is supported every step of the way, no matter how big or small your needs may be.

A Care Manager is a Registered Nurse that provides one-on-one care to help individuals reach their health goals.
Transition of Care Coordinators work with patients to reduce readmission to the hospital. If you are ever admitted to a local hospital, you may hear from our Transition of Care Coordinators. They will discuss aftercare of your recent visit, and ensure you understand your results from your visit, your medications, and you’ll work together to avoid another admission.

Care connectors reach out to patients after they have visited the emergency room. They will ensure patients understand any post visit summaries, and how to care for any injury they may have.

Community Health Workers work with patients to eliminate barriers to good health. If patients need assistance with insurance paperwork, signing up for insurance, access to food resources, or just a helping hand, Community Health Workers are available at each of our clinic locations.

Our Registered Dietician and Nutritionist sees patients in-person Monday, Wednesday, Thursday, and Friday in Lincoln and Tuesdays in Ossineke with all locations being serviced via tele-health.
The Alcona Health Center Mobile Unit is available to meet patients where they are. You may see this unit out doing outreach efforts in our communities, providing resources to those who are mobility impaired, and more.

Family Medicine Residency Program

The MyMichigan Midland Family Medicine Residency Program – Alcona Health Center Campus allows for an opportunity to truly experience the realities of practicing in a rural setting. The Residency Program is a total of three years. The first year is spent exclusively at the MyMichigan Midland location. For the following two years, residents split their time between seeing patients at Alcona Health Center in Ossineke and rotations at the newly remodeled MyMichigan Medical Center Alpena. The Family Medicine Residency Program launched with Alcona Health Center in June of 2022. Residents interact with a wide range of patients from newborns to geriatrics and from a diverse socioeconomic and cultural backgrounds. Fairly unique to the area is an Amish community who accesses care at the health center.

MIDOCs is a state- and federally-funded program expanding graduate medical education residency positions in select specialties to recruit and retain physicians in underserved areas of Michigan. Learn more at https://www.michigandocs.org/
