Primary Care

Alcona Health Center provides comprehensive primary care. Our team of nurse practitioners, physician assistants and doctors, along with other members of your care team will work with you to help set goals and help you to achieve your best possible health. We offer acute care for non-life-threatening illness or injury, preventative care including screenings and vaccinations, and care for chronic conditions or diseases including educational support and referrals to specialists or community resources when needed.

Medical Services

Physicals and Medicare Wellness Exams are an important part of your preventative care.  Your healthcare provider will address any concerns you may have and work with you on a plan for what screenings or vaccinations that you may need. Staff will coordinate your care; scheduling test or setting up referrals where needed.

AHC provides Well Child Visits and Sport Physicals. These services help to make sure your child is on track developmentally and the screenings done at these appointments can catch potential health issues early, leading to better interventions and outcomes.

At AHC you have a whole team of healthcare professionals who can assist you with your care. In addition to your primary care provider, we have Community Health Workers, Care Managers, a Dietitian, and Care Connectors who can support your needs. Whether you need assistance with health education, help coordinating appointments and tests with a specialist, or help getting to your appointments we are here for you.

Feeling ill? Have a non-life-threatening injury? We have some same and next day appointments reserved at each location. We want to make sure we are able to take care of you when you need us!

Alcona Health Center provides vaccines to anyone in the community who needs one.
