Tiger Health Extension at Alcona Elementary

Welcome to Tiger Health Extension, brought to you through a unique partnership with Michigan Department of Health and Human Services, Alcona Schools and Alcona Health Center! We are a full-service school-based community health center serving the medical and behavioral health needs of youth aged 3 to 21 years old using a Patient Centered Medical Home model of care. We strive to provide quality healthcare and excellent customer service. Established patients have access to a member of our After-Hours Care Team whenever the office is closed. This includes evenings, weekends, and holidays.
