About this Location
Welcome to Tiger Health Extension, brought to you through a unique partnership with Michigan Department of Health and Human Services, Alcona Schools and Alcona Health Center! We are a full service school based community health center serving the medical and behavioral health needs of youth aged 5 to 21 years old using a Patient Centered Medical Home model of care. We strive to provide quality healthcare and excellent customer service. Special appointment times for higher risk patients –
As part of our COVID-19 response plan, we have set aside special appointment times for the elderly or those with certain medical conditions that may make them higher risk for getting complications if they get COVID-19 or another virus. Please let the receptionist or scheduler know if you have: Asthma, Chronic Kidney Disease with Dialysis, Diabetes, a serious heart condition, a blood disorder or if you are immunocompromised.

Andrea Eby, FNP-BC
Andrea Eby, FNP-BC is a Nurse Practitioner at the Tiger Clinic. She graduated as a Registered Nurse from University of Toledo in 1999 before furthering her education online through the University of Phoenix, ultimately obtaining a Master's of Science in Nursing. She completed her post-graduate Family Nurse Practitioner certification from Saginaw Valley State University in 2012.
Kathryn Fouchey, MA, LPC
Kathryn Fouchey, MA, LPC is a Behavioral Health Therapist. She studied at Adler University and Western Michigan University, earning a Master's in Counseling with a specialization in Forensic Psychology.
