Care Coordinator – SUD Program (Alpena)


Alcona Health Center has a current opening for a Substance Use Program Care Coordinator at our Alpena location. The individual in this unique role will provide cross-system care coordination to assist the patient to access and navigate needed services.

PRIMARY DUTIES AND RESPONSIBILITES

Intake Assessment and Clinical Care

  • Assess patient and family’s unmet health and social needs.
  • Serve as a point of contact, advocate, and informational resource for patient, family, care team, and community resources and facilitate and attend meetings between patient, families, care team, and community resources, as needed.
  • Facilitate patient access to appropriate medical and specialty providers as well as other care coordination team support specialists.
  • Develop a care plan based on mutual goals with patient, family, and provider. Monitor patient’s adherence to plan of care and progress toward goals in a timely fashion, facilitate changes as needed.
  • Provide effective communication to clients, families, and other providers on program goals, legal and ethical treatment issues, diagnosis, treatment needs and goals.
  • Cultivate and support primary care and subspecialty co-management with timely communication, inquiry, follow up, and integration of information into the care plan.
  • Conduct spot checks of patients, including random pill counts and drug screens as directed by prescribing provider.

Communication and Health Information Entered into Patient Record

  • Enters appropriate treatment information into patient record in a timely fashion.
  • Progress notes documentation is completed accurately, is written and maintained in a manner that is clear, complete, current, and organized in accordance with state and federal regulatory requirements.
  • Progress notes entered into electronic record to insure appropriate ongoing patient care.
  • Continuously review charts for new and/or relevant information concerning clients.
  • Communicates patient’s health status to SUD Care manager to insure adequate provision of care.

Qualifications and Requirements

Licenses and Certifications

  • Possesses a current Medical Assistant Certification/Registration, Registered Nurse or Licensed Practical Nurse License.
  • Possesses a current Basic Cardiac Life Support (BCLS) CPR certification.
  • Possesses a current State of Michigan driver’s license and valid automobile insurance.

Technical Skills & Abilities

  • Ability to prepare correspondence and reports in Microsoft Word.
  • Ability to prepare and utilize Excel spreadsheets.
  • Ability to create, and send and manage email in Outlook.
  • Fully functional in the use of the Electronic Health Records system.

Alcona Health Center is currently recruiting for physician to join our Patient Centered Medical Home approach to quality care at our Oscoda, MI facility. We are seeking a MD or DO with a minimum of two years of experience to practice full service primary care with a solid team of clinicians. This is a Monday – Friday outpatient setting, with an “after hours” phone only call rotation.  Scheduling of a 4 or 5 day work week is flexible and negotiable.
