Alcona Health Center is currently recruiting for physician to join our Patient Centered Medical Home approach to quality care at our Oscoda, MI facility. We are seeking a MD or DO with a minimum of two years of experience to practice full service primary care with a solid team of clinicians. This is a Monday – Friday outpatient setting, with an “after hours” phone only call rotation.  Scheduling of a 4 or 5 day work week is flexible and negotiable.

Requirements:

Successful candidate will be a MD or DO with two years of experience and have a background in chronic disease management preferred, however all interested are encouraged to apply. Board Eligible/Board Certified in Family Medicine. Meet requirements for licensure in the state of Michigan.

Behavioral Health Therapist – Oscoda, MI


Alcona Health Center is currently recruiting a full time Behavioral Health Therapist to join our professional team in using a Patient Centered Medical Home approach to providing quality care. This position will be based in our Oscoda, MI clinic four days per week and work within Oscoda schools one day per week. Successful candidates should be comfortable in an independent setting and have a minimum of one to three years of experience in trauma focused therapy. Other job requirements include a familiarity with DSM-5, and a knowledge of electronic medical records and behavioral health billing. Must be a Licensed Master Social Worker, Licensed Professional Counselor, or PsyD to be considered for this position.

Registered Nurse – Substance Use Disorder Care Manager


Alcona Health Center is currently recruiting for the newly created position of Registered Nurse – Substance Use Disorder Care Manager. This position will be based in Alpena but will require some travel to other locations in Northern Michigan as needed.

Responsibilities:

In this role you will provide care management and care coordination for adult and pediatric patients who are identified for services in the substance use programs and have chronic health conditions and/or severe acute conditions, in the primary care setting, under minimal supervision.  The Registered Nurse – SUD Care Manager serves in an expanded health care role to collaborate with specialists, members of the health care team, and patients/families to ensure the delivery of quality, efficient, and cost-effective health care services.  They will assess plans, implement, coordinate, monitor and evaluate all options and services with the goal of optimizing the patient’s health status.  Successful candidate will have the ability to integrate evidence-based clinical guidelines, preventive guidelines, and protocols, in the development of individualized care plans that are patient-centric, promoting quality and efficiency in the delivery of health care.

Requirements:

Candidates must possesses a current Michigan state licensure to practice nursing and a current Basic Cardiac Life Support (BCLS) CPR certification.  You must possesses and maintain a current driver’s license and automobile insurance as travel to Cheboygan, Harbor Springs and other Northern MI locations will be required at times as needed.  A background working in behavioral health and/or Substance Use Disorder programs is helpful.  Upon hire the Registered Nurse – SUD Care Manager will be required to complete some SUD specific training, self-management support training and Motivational Interviewing training.
