We are currently dedicating certain hours of the day for WELLNESS APPOINTMENTS. SCREENINGS: We have implemented a front door screening program for anyone who enters the facility. The screening involves brief questions regarding your medical condition. We ask that only essential people accompany the patient to the clinic. DENTAL SERVICES hours have temporarily changed to treat emergent cases only by appointment. Our PHARMACIES are open. As a Federally Qualified Community Health Center, we are able to offer an income based Sliding Fee program to assist lower income families. You may pick up this application at your nearest location or print off the application form and list of supporting documents here. Your completed application can be turned in with the requested supporting documents to the location where you go for care.

Insurances We Participate With

Alcona Health Center participates with Medicare, Medicaid and many commercial insurance companies.

We will courtesy bill for any commercial insurance but recommend that you contact our Billing Department at (989) 736-8157 ext 9806 if you have questions about whether we participate with your plan. If you have questions specific to what your insurance plan covers it is best to reach out to your insurance company.

Payment

Payment is expected at the time of service for all patients. However, if you do have a bill that you owe on, we are able to take a payment with credit or debit card online.  Access our online payment feature here – Online Bill Pay  If you prefer to make a payment by phone you may call (989) 736-8157 and ask for “Billing”.
